Anyone can make a referral for Gilchrist Hospice Care – patients, family members, physicians, other health professionals, friends and clergy.
In most cases, patients will be admitted to the program within 48 hours after receiving the referral information. Gilchrist Hospice Care will complete the necessary paperwork and obtain physician orders to streamline the admission for the patient and family.
